The onset of heart problems alters a
person’s life in many ways. These times are difficult for both the patient and
the family. Healthy Hearts Cardiopulmonary Rehabilitation often begins in the
hospital after a heart attack, heart surgery or other heart treatment. It then
continues in our outpatient program after you leave the hospital. Healthy Hearts
includes exercise, education, counseling, and learning ways to live a healthier
life. Together with medical and surgical treatments, Healthy Hearts can help you
feel better and live a healthier life.
Participation in our outpatient program requires a
physician referral. Tell your doctor or nurse that you are interested in Healthy
Hearts and ask which rehab services or plans are best for you.
Once you are referred to the program, a team member
will contact you to schedule an initial interview and orientation session. In
certain instances, your insurance carrier may pay all or part of the cost of the
program for the initial rehabilitation phase.
